Responsibilities:

- Collaborate with team members, community partners, family members, and support networks to address individual needs and goals.
- Maintain professionalism, compassion, and respect when interacting with residents, clients, families, and stakeholders.
- Support individuals in achieving personal, social, educational, vocational, and healthcare-related objectives as appropriate.
- Complete required documentation, reports, and program records accurately and in a timely manner.
- Responsibilities vary based on the position, level of experience, licensure, and area of specialization.
