Responsibilities:

• Work directly with, support, and advocate for individuals within underserved and disadvantaged communities.
• Provide services, resources, and assistance that promote the well-being, independence, and success of program participants.
• Collaborate with team members, community partners, family members, and support networks to address individual needs and goals.
• Maintain professionalism, compassion, and respect when interacting with residents, clients, families, and stakeholders.
• Support individuals in achieving personal, social, educational, vocational, and healthcare-related objectives as appropriate.
• Complete required documentation, reports, and program records accurately and in a timely manner.
• Responsibilities vary based on the position, level of experience, licensure, and area of specialization.
